Immediate loading with a provisional complete maxillary restoration using guided surgery protocol

This study presents the immediate loading technique of a complete maxilla. Once the patient's case study is achieved, a surgical guide is performed with the NobelCinician (Nobel Biocare) software for implant planning.

This software assimilates mucosal and anatomical bone data as well as prosthetics, ensuring optimal implant planning virtually. The output renders a surgical and prosthetic guide and is used to craft the provisional prosthesis before heading to the surgery itself. Thanks to the output and to the prosthetic planning done by the dental surgeon, a guided restoration of the maxilla implant is possible. Crafted at an earlier stage thanks to the output, the provisional prosthesis is immediately installed for 6 months, guaranteeing the bio-integration and the optimal durability of the implants. Every step of the laboratory prosthetic phases are detailed in the article.

The article presents the prosthetic implementation of the restoration of a right maxilla incisor with a NobelActive RP implant. After stabilising the mucogingival environment with the help of the provisional prosthesis, digital photos are processed for the topographical survey of the shades and for other details. The customisation of the coping is imperative for the prosthetist's work. An advanced biscuit is used for clinical fitting and then a dental impression is performed for the laboratory. The single maxillary central teeth remains a great challenge when the two entities at hand are located several hundreds kilometres apart.

In the plenary room, Jean-Marc Etienne illustrated his presentation "The laboratory in the digital age" with 3 clinical cases of screw-retained prostheses.

In a rallying speech, Jean-Marc Etienne linked Digital with "sharing and speed" emphasizing the need to use these new tools intelligently while striving for the excellence they can bring us nowadays, against some tendencies which tend to redefine "standards" in a more mediocre way...
