The All-on-4®fixed implant-borne total rehabilitation procedure for the mandible

The All-on-4® treatment concept (Nobel Biocare) within the framework of fixed implant-borne total rehabilitation for the maxilla and mandible is a scientifically proven and clinically prescribed therapy.

The All-on-4® surgical protocol, developed at the end of the 1990s by Paolo Malo, provides simple and immediate full-arch rehabilitation for edentulous patients with 12 fixed teeth on just 4 implants, with immediate loading, no later than 24 hours after the operation.

In the clinical study chosen for this issue of the "IFCIA newsletter", we provide a stage-by-stage description of mandible rehabilitation development using a fixed prosthesis based on a simple, accurate and standard protocol and quick execution, restoring the patient's masticatory function and aesthetics.

Immediate loading of four or six implants in completely edentulous patients

In this retrospective study, 44 patients received screw-retained full-arch acrylic resin provisional prostheses connected on 4 or 6 implants in the mandible or maxilla, respectively. With a mean follow-up of 17.6 months, 3 of 205 implants were lost and replaced successfully. Cosmetic fractures were observed and fixed in six patients. 

The results confirmed that immediate loading of 4 mandibular or 6 maxillary implants with an acrylic resin prosthesis for full-arch rehabilitation is a reliable technique in the short- and long-term.

In this issue of the "IFCIA Newsletter", we are dealing with complete oral rehabilitation after the extraction of multiple teeth, placement of implants, MCI and prosthetic construction. The use of simple and accurate protocols enabled us to optimise the restoration of the masticatory function and the aesthetic requests of the patient.

Guided Bone Regeneration (GBR) technique applied to the restoration of incisor implants

Abstract:

The loss of a dental organ normally causes bone resorption, —a topic widely discussed in scientific and clinical journals. In order to optimise the results of both the biomechanics and aesthetics of the incisive section, reconstruction of the tissue lost is required. Both hard and soft tissue are involved.

Several techniques have been developed and applied; bone grafts, guided bone regeneration, vertical and horizontal osteotomies, connective tissue grafts to name but a few... with the single objective of getting closer to the initial state of the teeth and obtaining satisfactory, stable and long-lasting results.

Simultaneous extraction implantation, immediate aesthetics and guided bone regeneration

Résumé :

Une patiente est adressée pour un avis de réhabilitation implanto-portée en remplaçement des dents n° 24 et 25. Elles présentent des atteintes endodontiques et parodontales importantes rendant leur pronostic défavorable.

Le CBCT révèle un volume osseux satisfaisant ce qui a orienté notre choix thérapeutique, malgré la perte d’une table osseuse vestibulaire, vers l’option de l’extraction-implantation associée à une régénération osseuse guidée simultanée à une mise en esthétique immédiate.

Ce protocole permet de réduire considérablement le nombre d’interventions et offre un confort très apprécié par les patients.

Extraction, Immediate implant placement and loading in the maxilla incisor sector. Management of a fenestration with guided bone regeneration (GBR).

Abstract:

Immediate extraction implantation is a technique which has been used and approved with a clinical track record. Loading immediately after the implant is put in place is provided by a provisional crown. In the anterior sector, this approach has several advantages: on one hand, it provides the patient with significant aesthetic comfort and psychological ease, and on the other hand, on a morphological level, it helps to control the peri-implant mucosa healing process and maturation by preparing the site for pleasant and stable aesthetic outcomes over time.

In this clinical case, the stages of replacement of a central incisor presenting inflamed periradicular lesions of endodontic origin and bone fenestration (vestibular) will be described. The stages of extraction and immediate implantation associated with guided bone regeneration, connective tissue grafts and the placement of a immediate provisional crown have also been extensively described.

Aesthetic implant rehabilitation in the posterior maxillary region: a clinical case with significant osseous loss

Abstract:

How does one bring together high aesthetic demands and implant rehabilitation in the posterior maxillary region when faced with a terminal osseous loss and a generous smile?

In this clinical case we present an example of original temporisation with the help of a provisional implant, well performed tissue reconstruction with the help of sinus floor elevation, guided bone regeneration and mucosa adjustment, leading to very good biological integration of the prosthetic restoration.

Onlay bone grafting: when and how?

Summary:

There are many advantages to the bone grafting coffering technique which requires autogenous bone grafts taken from a ramus area with a resorbable membrane. Bibliographic support and clinical experience in this technique show that the risk of bone resorption is reduced and that there are minimal complication risks. In addition, the bone quality attained during implant placement can reach a good primary stability with an optimum positioning in a second phase. This technique remains dependent on the experience of the treatment team and should only be chosen after evaluating other treatment options.
