Special edition of Le Fil Dentaire: "Bone management in implantology"

In this special edition of Le Fil Dentaire published in September 2017 on "Bone management in implantology", we have included numerous clinical cases which in order to cover the most common approaches in our daily work, based on a well-defined therapeutic gradient.

Emergencies in Periodontics

The term "emergency" describes the onset of a painful, acute phenomenon requiring a precise diagnosis so it can be treated quickly and correctly. This last point is particularly important when dealing with a periodontal emergency, in which gum pain is often associated with extensive tissue impairment with a high risk of deterioration. Among periodontal diseases, periodontal abscesses and necrotic periodontal diseases are among the most frequent reasons for emergency consultations. Nevertheless, even if their prevalence is minimal, certain gum diseases which are not induced by plaque, are also characterised by intense pain, which, beyond the periodontal repercussions, may also compromise the patient's health considerably.

Implant insertion after tooth extraction: clinical outcomes with different approaches

Authors: Jeremy Abitbol, Degorce Thierry, Hadi Antoun

The "socket shield" technique seems beneficial to the patient. It incorporates the benefits of immediate implant placement with provisional restoration. The patient undergoes one intervention and benefits from fixed implant-supported prosthesis on the day of the surgery.

Abstract

Background: Few large-scale follow-up studies are reported on routine implant treatment.

Purpose: To report retrospective data on early inflammatory and early implant failures in a large number of routine patients at one private referral clinic.

Is there an increased risks of developing periodontal diseases when suffering from psoriasis?

Author: Ons ZOUITEN

Summary:

Psoriasis is a frequent, chronic erythemato-squamous dermatitis that affects around 2% of the population and its cause is unknown. This autoimmune disease with an inflammatory component is mainly reflected in the skin. Its pathogenic effects are systemic. Usually a benign disease, it can be serious when dealing with psoriatic arthritis or pustular psoriasis.

Oral symptoms have been described for many years, such as the geographic tongue, cheilitis, and, as early as 1992, Yamada (1) discussed a possible link between periodontitis and psoriasis.

Current data on the evaluation of soft tissue within an Extraction Immediate implant placement and Temporisation protocol in the anterior sector.

Even though the prospect of an extraction, immediate implant and temporisation (EIIT) in the anterior sector is a challenge for practitioners when such a clinical situation arises, for the patient this technique entails a positive psychological impact since aesthetics and comfort are immediately obtained.

The patient, comforted by an improved smile and relationship with others, adheres easily to the treatment especially since the transitional stages require temporisation and validation periods, the duration of which is dictated by precise biological rules.

As part of his University Degree in Surgical and Prosthetic Implantology, the author of this paper re-examines this subject in the light of current literature.

“Is the guided bone regeneration of the posterior mandible reliable and stable.” Review of the 2015 ADF Congress

Implant treatment in the posterior mandible presents many difficulties. Its anatomical characteristics and its reduced vascularisation can lead to failure. Its centrifugal resorption can cause bone defects in both the horizontal and the vertical planes. Therefore, it is essential to know the physiology of the osseous tissue and the mucous membranes at the mandible, along with any anatomical obstacles prior to placing implants or starting a bone augmentation.

Preservation of the alveolar ridge in the posterior maxillary region

Authors: Sylvie Pereira - Co-authors: Bouchra Sojod and Hadi Antoun
Journal and references: Objectif Paro-Décembre 2014-N°36

Within the limits of this retrospective clinical study, the conclusions show that the socket filling can increase the probability of preservation of the alveolar ridge and / or prevent sinus pneumatization. Therefore, maintenance of the alveolar ridge in the posterior maxillary area can reduce the indication for a sinus graft. Moreover, the relationship between the sinus floor and the topographic position of the teeth to be extracted could play a role in the decision-making concerning implant placement with or without sinus graft. A sinus graft seems necessary in most Class 4 cases, when the roots are in close proximity to the sinus.
